New Patient Registrationregister_now

The practice welcomes all patients residing in the practice area.

To register you need to complete a 2020 New Registration Form and produce some form of photo identification (ID), as well as proof of address.  These could be a passport and/or driving licence as well as a utility bill and/or bank statement.

This form can be used for Stanford Medical Centre, Islingword Road and Cockcroft Surgery (University of Brighton students)

Patients from outside the UK will also need to complete the NHS England registration form GMS1 form. Please ensure you complete the rear section titled 'supplementary questions'

Medical treatment is available from the date of registration. Please contact reception for further information.
