New Patient Registration
The practice welcomes all patients residing in the practice area.
To register you need to complete a Registration Forms and produce some form of photo identification (ID), as well as proof of address. These could be, a passport and/or driving licence as well as a utility bill and/or bank statement.
This form can be used for Stanford Medical Centre, Islingword Road and Cockcroft Surgery (University of Brighton students)
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.
